TULSA, Okla. (AP) - Oklahoma health officials and a restaurant at the center of this summer's deadly E. coli outbreak have signed an agreement to reopen the eatery.

The two-page document, obtained by The Associated Press through an open records request, lists several conditions that must be met before the Country Cottage restaurant in the small town of Locust Grove can reopen.

They include disconnecting a private well on the property in the northeastern corner of the state, allowing for environmental testing in the restaurant upon request and implementing a monitoring system for employee hand-washing.

The August outbreak became the largest in the nation's history for the rare E. coli strain O111, killing one Ardmore-born man and sickening more than 300 people. Investigators have yet to identify the exact cause of the outbreak.
