CVR Energy Inc. completes investigation into fatal blast

WYNNEWOOD, OK - CVR Energy Inc. completed their investigation into what caused the fatal blast at the Wynnewood Refinery in September.

The company's Vice President of Corporate Affairs Angie Dasbach said the blast was caused by human error and inconsistencies with the standard operating procedures and operator training.

“We’ve developed a list of corrective actions that are being implemented for the whole facility," Dasbach said.

CVR Energy's investigation found no mechanical issue with the boiler that exploded.

The blast killed two employees, Russell Mann of Davis and Billy Smith of Pauls Valley. Dasbach said the two men had nothing to do with causing the explosion.

Occupational Safety and Health Administration is now investigating.
